Health Home Care Manager
We'll start by answering your first question,
what is a Health Home? Well ironically enough, it is not a home in the physical
sense of the word. A Health Home pertains to service coordination efforts to
help those in need obtain the services they require as Medicaid recipients. As
a Care Manager you will work with consumers in order to map out their path to
better health. Recipients of these services receive help from Care Managers in
managing one or more chronic and/or mental health issues in the realm of
working collaboratively with providers and promoting primary and preventative
care in order to reduce or eliminate the need for a hospital visit. A
comprehensive approach is applied to these services in that throughout
concentrated communication channels amongst staff members and consumers, a
common goal is achieved, and that is improved health and well-being.


Please make sure you
have: A Bachelor's Degree in Human Services and a minimum of two years
experience working with individuals living with mental illness, substance abuse
or other disabilities. proficiency with computer software such as Microsoft
Office Suite and other web-based programs, reliable transportation, and a
current New York State Driver License in good standing. Incumbent must be
proficient in documentation and writing skills.


It is helpful if: You
have prior experience with the Medicaid delivery system.
